By submitting this form and participating in any training session offered by ALC Goalkeeper Academy ("Academy") and/or Luis Alvarez ("Coach"), you (the parent or legal guardian) acknowledge and agree to the following:

1. Assumption of Risk: You acknowledge that soccer goalkeeper training involves physical activity and inherent risks, including but not limited to physical injury, strain, bruising, or other harm. You voluntarily accept these risks on behalf of the minor participant.

2. Release of Liability: You hereby release, waive, discharge, and covenant not to sue ALC Goalkeeper Academy, Luis Alvarez, and any affiliated coaches or training partners from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury sustained by the participant while engaging in Academy activities.

3. Medical Authorization: In the event of an emergency, you authorize the Coach to seek immediate medical attention for the participant. You represent that the participant is in good physical health and capable of participating in athletic training.

4. Media Release: You grant permission for ALC Goalkeeper Academy to photograph or video record training sessions for educational or promotional use on social media and website platforms, unless explicitly requested otherwise.

5. Indemnification: You agree to indemnify and hold harmless ALC Goalkeeper Academy and Luis Alvarez from any claims arising from the participant's conduct during training activities.

This waiver is governed by the laws of the State of California. By checking the box below, you confirm you have read, understood, and agree to all terms above.
